SAKA Standing Order Form

Another way to make regular donations to SAKA, renew membership or become a new member is by downloading and completing the SAKA Standing Order Form and sending it to our Treasurer via the freepost address details given on the form.
NB: It is not recommended to send documents that include bank details via email which is unsecure.
SAKA strongly recommends that you download and electronically fill out this document, print it out and return it via the freepost address.


Please carry a donor card

Remember - organ donation is the gift of life. Please sign up to the organ donor register, carry a donor card and tell your relatives your wishes.

Cards are readily available from your GP or chemist or by contacting the Organ Donor Line on 0845 60 60 400. You can also register quickly and simply online at www.uktransplant.org.uk.
